CCM Guidelines used for working on your file:
The Adjuster should fax the first report of injury to 501- 455- 2333 or call 501- 455- 5333with the information.
I will contact the adjuster the same day for direction and to obtain the medical records.
Contact with the claimant will be attempted by telephone upon receiving the file and an initial contact letter will be sent to the claimant within 24 hours.
I will gather any missing records, review the file and give the claims adjuster my assessment of the file within the first 2 days of receiving the file.
I will make every effort to attend all physician appointments with the claimant to assure the claimant and claims adjuster understand the treatment plan and projected length of the treatment.
I will attempt to anticipate upcoming treatment options and obtain approval from the claims adjuster prior to the appointments to save time on scheduling and waiting.
Reports to the carrier will be every 30 days, following any activity of the file, or as requested by the carrier. Reports are concise and informative, not lengthy. Most reports will be charged as a half hour. If you prefer more information in your reports, that can be accommodated as well. All correspondences, medical records, therapy notes, etc., that were collected during that reporting period, will be enclosed with each report. Reports are emailed, faxed or mailed, at your preference.
Billing will be emailed immediately following the report submission. Billing guidelines for your company can be discussed at the time of referral.
Case Management charges can be discussed at anytime.
Prior to every appointment, I will review the entire file, make notes and write out questions that will need to be answered by the physician. I will gather work releases and other information that will have to be filled out and signed at each appointment. I will attend each and every appointment completely prepared and completely up to date on all occurrences with your file. You will be updated immediately following each appointment.
